Directions: The first screen will prompt you to log in with your UCINetID and password.
Upon successful log-in, you have four options:
Electronic modification requests are stored for 90 days and then deleted from the web module if not submitted to the IRB. Create, review and submit or retrieve an Electronic Modification (e-MOD) Request UCINetID: To create or submit an Electronic Modification Request, users must log in with a UCINetID and password, which is used to authenticate (i.e., it serves as your "electronic signature") that the information is provided to the IRB by an authorized person. For information about Obtaining/Activating a UCINetID, click here. Note: Although any member of the study team may create and save an e-MOD request, ONLY THE LEAD RESEARCHER (LR) OR FACULTY SPONSOR (FS) may submit the request to the IRB for review. |
